- Future Plans (Business Expansion, and etc.)
- For Franchise / Foreign Brands
- Letter of Authority from the Principal
- Product and Store Identification
- Concept of the Store
- Major Differentiation
- Major Product to be sold and its approximate price
- Target Market
- Architectural Perspective for the Proposed Outlet
- Utility Requirements (Electrical, Water, Exhaust, etc)
- For Applicants with existing outlets in major shopping malls: Photographs of the existing outlet(s)
